Once you’re enrolled, you only pay a $10 copayment for your annual eye examination, when you use a network provider, and you can visit any eye doctor in the EyeMed Network. Here are more of the plan’s features when you take advantage of the EyeMed Network:

• Low exam copayments
• Single-vision eyeglass lenses are covered at 100% after a $25 copayment
• $100 retail allowance for frames with 20% discount on balances over $100
• Medically necessary contact lenses are covered at 100%
• $115 allowance for elective contact lenses with an additional 15% discount on balances over $115
• Discounts on Lasik
• 20% off retail on any services and materials not covered by the plan
• Standard polycarbonate lens available at no charge to dependents under age 19. All other members pay fixed copayment of $40
